Financial Policy

As a courtesy to our patients, we will file insurance claim forms with your carrier on your behalf. However, your active participation in the insurance claims process may be required. We recommend that you confirm that your insurance will cover services provided by our practice before your appointment.

Certain tests such as a treadmill test, Holter monitor, echocardiography and other special tests are often required in the diagnosis or treatment of cardiac conditions. When these tests are required, they will be charged separately from your office visit. Fees for certain procedures do not include follow-up visits, and will be charged separately. For convenient reference, you will receive an itemized receipt with a breakdown of all charges.

Medicare Patients

As a participating provider of Medicare Part B (Physician Services), we will only charge you for your Medicare co-insurance, deductible, and any services rendered but not covered by Medicare.

Commercial Insurance

Please remember that your insurance contract is between you and your insurer. If your insurance company pays only part of your bill, or rejects your insurance claim, you are financially responsible for the balance. Payment is expected upon receipt of your statement. Please bring your insurance cards and any referral forms with you each time you visit. Payment for co-pays or co-insurance is required on the date you receive the service.
